(the “Activity Provider”)

1. Acknowledgement and Assumption of Risk

The undersigned acknowledges and agrees that participation in the activities offered by the Activity Provider — including but not limited to kickboxing, grappling, wrestling, fitness training, and related combat sports — involves inherent risks of injury, illness, or even death. These activities may include sparring, drills, use of fitness equipment, and other physically strenuous activities.

2. Release of Liability

In consideration of being allowed to participate (or for the Minor Participant to participate), the Participant and/or the Parent/Guardian, for themselves and their respective heirs, assigns, administrators, and legal representatives, hereby release, waive, and forever discharge Icon Combat Inc., its owners, employees, officers, agents, affiliates, successors, and assigns from any and all liability for any loss, damage, injury (including death), or expense that may be suffered, directly or indirectly, as a result of participation in the activities, including due to the negligence of the Activity Provider.

3. Fitness to Participate

The undersigned confirms that the Participant or Minor Participant is physically fit and has no medical conditions that would affect their ability to safely participate in the activities. The Participant or Parent/Guardian agrees to consult a physician before participation if needed.

4. Legal Capacity

• If the Participant is 19 years of age or older, they confirm they are legally competent to sign this Agreement.
  
  

• If the Participant is under 19 years of age, the Parent/Guardian acknowledges they are legally authorized to sign on behalf of the minor and voluntarily assume all associated risks on the Minor’s behalf.
  
  

5. Indemnification

The Parent/Guardian agrees to indemnify and hold harmless the Activity Provider from any claims brought by or on behalf of the Minor Participant or any third party, related to the Minor’s participation.

6. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the Province of New Brunswick , Canada.

7. Full Understanding

The undersigned confirms they have read and understood this Agreement in full, have had the opportunity to ask questions and obtain independent legal advice, and are signing voluntarily.
